Great to have
If you are not at home where your DNR form is displayed and you are in an accident without this form, the medics will try everything to resuscitate you. This included breaking all ribs if necessary. I don't want this to happen so I got this card.

Perfect, just one laminating tip
I did one card each way with the laminating. The one I put in with the writing to the rigid side came out better. The red side is brighter against the adhesive and the writing side is clearer without adhesive.

Not sure how to test this, then review.
Got it for myself. There were no instructions on how to test the card then write a review about it. Amazon reviews from the grave?What I can say is that it is a bit too big to fit in a wallet, like behind your driver's license.
